Pruritic urticarial papules and plaques of pregnancy. That’s what PUPPP stands for. PUPPP a skin condition that only occurs during pregnancy, almost exclusively on a first pregnancy, and usually when that first pregnancy is a boy. In short, it’s a rash that develops on the belly and spreads to the legs, chest, and arms from there. It’s not just any rash, by the way. Apparently it’s the most ridiculously itchy rash that anyone has ever experienced, it’s extremely annoying, highly uncomfortable, and lasts until just after giving birth. There is no known cause or treatment for PUPPP. Other than driving women stark raving-mad, it’s completely harmless to the woman and the baby. Of course harmless is relative statement, as some women actually are driven to asking to induce at week 37 just to get the suffering over with.
PUPPP affects roughly 1 in 150 pregnancies and usually starts somewhere around week 37, meaning the misery is usually short-lived. My lovely wife, being the self described over achiever, started showing symptoms right around the middle of week 19. It started out as something like a heat rash on the upper part of her belly. After a couple of days the heat rash turned into dark red splotches with raised bumps, and the itching began. A phone call to the doctor’s earned an ominous “oh dear” reaction from the nurse. The subsequent visit with her doctor’s Certified Nurse Midwife went just as well. She took about .3 seconds to look at The Pregnant One’s belly and immediately took a step back while inhaling through her teeth making that “yikes” sound. Yep. It’s PUPPP’s. And a really early case of it too. But, you know, it does go away after pregnancy. 22 weeks from now.
Here’s where we take a little sidetrack. Before we went in for her appointment, I had spent the prior couple of days doing a ton of online research into PUPPP, as I was pretty certain that’s what she had. My search led me to this excellent resource which sent me off in a million different directions. Two things I found pop up everywhere where dandelion root extract (a liver detoxifier, as most skin rashes are actually liver related) and Ranier Soap Works PUPPP Relief Soap and Cream. Acupuncture has also mentioned as a possible aid in treating PUPPP.
As it turned out, the nurse also being a midwife worked for us. While she was not familiar with the use of dandelion root during pregnancy, she uses herbs in her other practice and didn’t freak out on us when we asked about it. Instead she pointed us in the direction of an acupuncturist and herbalist that specializes in women’s reproductive health. If he says that we’re cool, then she’s fine with it (unofficially, of course). After a phone conversation with the herbalist, we’re given the green light on dandelion root and give it a shot.
Now here’s the thing… at this point we’re kind of throwing the whole kitchen sink at the thing. We’re using the soap, a soothing cream that makes the bathroom smell like Doublemint Gum, and the dandelion root (organic, even) extract pills, so it’s hard to really narrow down what is or isn’t working. The only clue we have is this: in places where I’d read about the dandelion root extract it says you won’t notice any affect until about the 5th day of taking it. Low and behold, on the 5th day she wakes up and the rash is fading. By about the 10th day the place where it started has returned to almost normal, the places it had spread to are fading, the signs of spreading have stopped in their tracks, and the itching is gone. The stuff freaking works!!
We just had our 20 week appointment with her doctor and both quietly decided we would just keep this to ourselves. It works, it’s safe, and frankly that’s all that matters.
p.s. A special bit of husband/fatherly angst to the ultrasound tech who went into the “oh, your baby is still breech and you know that’s an automatic c-section if he doesn’t turn” routine yesterday. Here’s a concept: We’re only at 20 weeks. You’re not a doctor or a nurse. You’re an ultrasound tech. The nurse told us not to worry about it as we’re pretty early along, and yet, you just had to throw that little nugget of information in there to add to the normal everyday worries that come along with being pregnant. How about you just take your pictures and STFU. Thanks.
So this is what the books mean by going into “protective husband mode”.
edit (4/29/08): This post has received a fair amount of traffic (primarily from Google searches), so I thought I’d update for anyone that’s at all curious. We are at 29 weeks now and everything is going swimmingly as far as the PUPPP is concerned. The Pregnant One actually stopped taking the dandelion root extract about 3 weeks ago as the PUPPP rash has entirely vanished. Really. Much to the wonder of a medical practitioner. She still uses the Ranier Soapworks PUPPP soap on occasion, just to make sure and because it smells nice and leaves her skin soft (albeit a bit dry). So, in our instance the dandelion root really seemed to be just what was needed. I hope this helps anyone that is reading this looking for treatment ideas.
Post Script: While I haven’t updated this blog in some time, I am receiving a lot of traffic, and quite a few comments on this post in particular. Most of the comments are questions about the dosage. This is where I have to be frank and honest… I’m not a doctor, herbalist, mid-wife, or any other sort of certified expert in that field. The fact is, I don’t feel comfortable sharing that information, because I don’t want that responsibility. I’m sorry for those that may not think that’s right, but the information is out there. My advice? Find an herbalist or midwife that can answer those questions for you. Don’t just take the word of some dude on the internet. I was lucky enough to find one for my wife that specialized in women’s reproductive issues and was kind enough as to take a little time on the phone and offer some free information. Call around and see what you find.

I suffered PUPPP during my 1st pregnancy-a girl, it began at 37 weeks and was unbearable, I tried all the recomended oils, creams, tea, dandelion, everyhting for weeks, nothing worked, I was placed on sleeping pills by week 39, until I had my daughter….April 2005.

Please could you tell me what dose of dandelion root your wife took? I have 520mg tablets and on the bottle it says one or two tablets a day – but I’m pretty sure I need to take more to make a difference to this itching.
Like your wife, I have an early case at 22 weeks, and am just keeping my fingers crossed that it will burn itself out rather than last until the day the baby pops out! I would seriously go insane if I thought I had to endure this for another 18 weeks.

Hi there! I found this post very helpful. I have PUPPPs as well and it started at 15 weeks! Needless to say I’m not giving up on a cure because I dont think I can endure this for another 25 weeks. I’m using the dandelion root and the Rainier products are in the mail. In the meantime I’m using a combo of calamine at night and aveeno during the day. I noticed that the red splotches have some darker flatter areas within them. It almost looks like the rash is drying up since those areas are not as swollen. Is this what you saw on your wife as hers started to fade? I’m wondering if mine is getting better or worse. It’s still terribly itchy though.

I am also a PUPPP sufferer! I had my first round with PUPPP at 22 weeks; it had cleared up by the time I was 28 weeks after using Grandpa’s Pine Tar Soap (made it worse before it got better) and 1 capsule of Zyrtek with 1 capsule of Pepsid every night before bed.
But now, 3 weeks later, it is coming back! I have tried my old remedies from above, but it doesn’t seem to be working. I’ve purchased some organic dandelion root capsules and dandelion root tea bags from my local natural foods store, but I’m not sure how much to take??
I’ve also read 2 T per day of flaxseed oil with the dandelion root should help, but I’ve read contradicting reports about its safety during pregnancy.

Hey there you nice dad! This is my third pregnancy but it is twins and I am at 23 weeks. The docs were very reluctant to believe I have PUPPP as it began on my feet and legs and now is spreading to my arms. But the dermatologist biopsied it, and that is the diagnosis. It is horrific!! I am scarring myself and my three year old keeps pulling my hands away saying, “Mommy, you will make yourself bleed.” Bless him. What is the dandelion dosage that worked for your wifey? I am ordering the soap now!!!

Ugh, PUPPP is the worst! I started to develop it at 32 weeks, was induced at 38 weeks because it was so unbearable, and now 3 weeks postpartum, it’s still there (and isn’t getting much better yet)! The worst thing is that it’s kind of a weird PUPPP/eczema hybrid, and my dermatologist says he hasn’t seen this very often (and he’s been in the field for 30 years). Does the dandelion root stuff actually work? I have a bunch of topical steroid creams, but really, ice packs have worked the best for me! Could someone chime in and let me know when your PUPPP actually cleared up for good? (I need some hope!!!)

Hi! I feel for all moms who had and have this horrible rash. For my case I had it on my 24th week. And I have spent $$$$$$$$ searching for cure. I hve used so many things on my skin already that I don’t know which really worked. I used DERMAVEEN LOTION which is really good for delicate skin. Have taken chlorpheneramine which my ob prescribed. THe rash went of fortunately but only for 4weeks!I had it again on my 32nd week! I am so desperate to get rid of this horrible rash and until now I am 8weeks postpartum and still have it!Will it still go?I need some hope. Have tried dandelion but no effect.Or should it be pure dandelion extract pill because the one that I was taking before was with some other herbs in.I pray that everymom will find cure and be relieved as soon as possible.

I just wanted to say thanks for creating this blog post — it gave me a laugh that I needed as I suffer through this and search online for anything that will help!! I was so lucky to get this at 14 weeks (and this is pregnancy #2) and my doctor even laughed today and told me to start using my power of prayer to make it stop itching. My prescriptions DO help (creams, etc.) but like all good things it wears off. My husband wants me to try the dandelion but I’m wary from reading mixed opinions about it working…Dosage would help? My OB just put me on super dose of steriods. I have been fighting PUPPS for 11 weeks. It seems to be working…I am barely itching. I am taking 60mg for three days, 40 mg for two days, and 20 mg for two days. He has found this to be very successful for his patients who are stuck with this itchy mess.

PUPPPs has been the most challenging thing I’ve ever experienced. Yet it makes me feel better to know that I am not alone. I too have tried EVERYTHING. I’ve spent hours on the internet researching, and have spent lots of money on creams, lotions, and pills that only semi-relieve the itching for a short time. I’ve never cried this much in my whole life — I feel like a lunatic, sobbing while taking cold showers and oatmeal baths and carrying around lotions and icepacks everywhere I go. I’m ready for a CURE! Mine began at 30 weeks with the worst outbreak at 32 weeks. My doc told me I could take Zyrtec in the morning, and Benadryl as needed in the evening. But I hate taking these while I’m pregnant. Does this Dandelion Root work?

I am suffering from this horrible PUPPPs rash it is my first pregnancy and it has started at 14 weeks, (so extremely early) the thought of having it until birth is horrific. I have spent £££ searching the internet for creams and oils along with my doctor. I live in UK and it is unheard of here. I went to A&E and saw 2x nurses and 3x doctors who were all clueless to what the rash was.
Finally after 2 weeks of complete discomfort and distress over this rash I found my cure to be. ‘Grandpas Wonder Pine Tar Soap’ following by Aveeno Oatmeal bath oil and and moisturizing cream called Cetraben (can be used wet or dry) using this morning and evening after a bath or shower. After 2 weeks the rash is clearing up, and I can hopefully enjoy my pregnancy. Another cure I feel for all those women out there that are unlucky to get this rash, as it is the most unbearable itching I have ever experienced.
When the rash was at its peak Calamine lotion was a temporary relief for the itching and burning of the skin. But don’t use it too much like I did as it dries out your skin, which makes Puppps worse. Keep your skin moisturized.

Thank you SO much for posting this. I was dieing at week 30 with PUPPPs. My dr. said there was nothing I could do. It was all over my belly and I could see that it was starting to travel to other areas. I couldn’t function at work or in meetings, I was going crazy, and no lotion would work for more than 30 min. At night, I was up every other hour applying Sarna lotion and cold packs to my stomach.
So after reading your blog I went to a herbalist who said take dandelion root capsules (I didn’t mention it, she said the only herb she would recommend during pregnancy is dandelion root). Day 5 I started noticing the hives were getting smaller and drier, now it is day 8 and my belly has no rash at all, just very dry rough skin which is discolored from the hives, and itching associated with that, which is totally bearable. I developed PUPPP at about 38 weeks and spent the first 3 days putting oatmeal paste, baking soda paste, and tons of Aveeno lotion with oatmeal on my belly. At a certain point, particularly as it spread to my thighs and hands and ankles, I just couldn’t take it anymore.
I went to the store and got Grandpa’s Pine Tar Soap after seeing this webpage http://www.tcinternet.net/users/kritzerburke/naomi/PUPPP.htm.
The Pine Tar Soap is a MIRACLE. Some people on here said that you should keep the PUPPP moisturized? I haven’t found that to be true. When I moisturize it makes me itchy. The Pine Tar Soap sort of dries out the pustules and stops the itching for me for 2-3 hours at a time. I have noticed a definitely improvement in the 2 days I have used it. Some people also say they took showers 2-3 times per day–I’m washing with this stuff every couple of hours with a washcloth, making sure I get a good lather before I rinse it off. IT IS A MIRACLE.
RE: the Dandelion Root, there should be dosage instructions on the bottle (there are on mine). I’m on day 3, and if it really takes until day 5 to see this rash start going away, I’m a very HAPPY camper.
I feel for the women who get this early. This is truly torture.

Thanks a lot for your post. I developed this horrible itch rash around week 28 and its been since getting worse….I am on week 32 now and feels like I can’t take anymore. My dermatologist gave me topical steroid and told me to take zyrtec everynight..but that only helped for a week…it did dry up the itch patches in some places…but I keep getting more… I have read about dandelion root on a lot of different sites, but some say its not good to be taken during pregnancy. When nothing was permanently working, I took the courage to start taking it 3 days back..so far I tool 3 pills. (I take 1 pill a day each pill is 550 mg)…I am yet to see the complete benefit..as last night I hardly slept 2 hours due to bad itching..I am still applying topical steroid but stopped zyrtec as I did not want to combine it with dandellion root..I keep getting worried while taking it..thinking it might be harmful for the baby…but reading your post gives me a lot of confidence…Could you tell me the dose that you wife was taking and if 550 mg tablet, once a day is enough..thats the dose written on the GNC bottle that we got…Please help…I also order the rainier soapworks cream but it has not arrived yet.

I started with PUPPS at 19 weeks and still had it for 5 weeks after the birth of my daughter. It was horrible, but it to will pass. I tried steriods, cream after cream, zyrtec, pepcid, pupps soap, dandelion root, flaxseed oil, sarna lotion…….I tried it all. My doctor did put me on ambien which helped me sleep (versus itch) at night. It is a horrible thing to have, but it is worth it in the end. At about 3 weeks after I had my daughter, the dermatologist gave me a steriod shot and that was my miracle…but they would not give me this until I delivered the baby and stopped breastfeeding. My heart goes out to you!

So sorry to hear that you started PUPPS at 19 weeks…I thought I was unfortunate to start it so early…I really feel for you….and I am scared to hear that it stayed for 5 weeks after you gave birth…Most people say that it goes away immediately after birth….so was it as awful even after delivery? How did you feed or do anything with this horrible itching…Did you take dandelion root capsules? What dosage did you take? I have 550 mg tablets and right now I am taking only 2 a day…wasn’t sure if too much of it might cause harm to the baby or something…can you tell me how much you took per day? How did you make it through those 21 weeks? Its hard for me to even start thinking…right now I am at 32 weeks and I am ready to deliver…My OB told me if I was miserable they can put me on oral steroids but I was trying to avoid that….

I did take oral steriods….I think two doses total. They did help for about one week. In regards to scars….I do have them and they look like chickenpox scars. I just itched too much and have scars on my belly, arms, and back. I hope they will fade over time and have considered getting Medirma for the ones on my arms. I did not get stretch marks, so I guess scars from PUPPS was my trade off. I took 3 dadelion root capsules a day. I didn’t think they helped much. I found that I itched less on days that I loaded up with water…strange, huh? You will get through it…I promise! I also only wore cotton clothes and stripped down to nothing when I got home each night. Do everything you can to stay cool…heat always made mine worse. I also used Aveno bath wash..which helped a little at night (along with a cool shower). Ambien allowed me to get rest which was so important with my mood towards the end of my pregnancy. I promise you…..it does go away!

I’m 21 weeks pregnant and I’ve had PUPPP for 5 days. This really is unbearable! I don’t know how I’m going to manage going the entire 2nd half of my pregnancy itching! I’ve tried all kinds of ointments and creams and the ONLY thing that has given me any relief is Sarna lotion. But that only seems to work well (few hours relief) right after a lukewarm oatmeal bath. I’m going to try the dandelion root asap. My OBGYN said there isn’t enough research on taking it to determine if it’s “safe” during pregnancy, so taking it would be up to me. I’m also getting the Grandpa’s Pine Tar soap to see if that will help too. I’ve been crying all day because this is making me so miserable! I can’t sleep for more than an hour or so at night because the itching gets 10x worse. I just want to have a cute pregnant belly, but with it looking like this (and it gets worse daily), I don’t want anyone to see my rashy skin!!! I’m so upset that I’m in that lucky 1% of pregnant women to get it!

I am 39 weeks and came out in this terrible rash 3 weeks ago…. It was just a bit itchy to start with, but the itching got unbearable . I have tried so many different things, calamine lotion, calamine cream, garnier soap and scent free bath oils, frozen veg (this stopped the itching… but found my legs froze for days – like bad circulation).
I couldnt contact my midwife at the beginning, so contacted my maternity unit, they advised me to go down and get checked because of the itchyness, they did liver function tests and everything was fine, told me to go str8 bak if the itching got worse. A few days later it had spread everywhere, still couldnt contact midwife, so called the maternity unit again and they said not to go down incase its anything contageous, and to see my GP str8 away. The surgery was shut so called out of hours GP (which actually turned out to be my GP)…. and what a help he was, didnt even want to see me, just said that its PUPPP and didnt want to give me anything for it. A few days later i noticed my legs and stomach going blue, couldnt get to see my GP or Midwife so went to A&E, Sat there for 3 hours for a doctor to tell me he had never seen anything like it and didnt know what it was (even though i had told him my GP had diagnosed PUPPP over the phone). He eventually gave up and sent me to the other side of the hospital where there was an on call GP – he too said he didnt know what it was and had never heard of PUPPP, but eventually prescribed me a steroid cream, which i was to put on twice a day for 2 weeks, needless to say it was a tiny tube, which lasted 2 days, because of the extent of the rash! I am now covered from my feet to my shoulders and i am continuously itching! Its so much worse when its warm, i havnt slept for 2 weeks and have spent every night on the sofa, because i tend to scratch more when in bed, i even woke up with my bed covers covered in blood from where i have scratched.
Thanks for the advice onn the dandelion root extract, i am off down the local health shop now to see what i can get. I really hope this works as i am goin out of my mind not sleeping or eating. I know i only have 6 days left before my due date, but i am seeing my midwife tomorrow, and i am going to plead with her to get me induced. Wish me luck!

I had PUPPs on my first pregnancy (2 years ago), it started at 30 weeks, i took dandelion herb tea 3 times a day and also applied the rainer soap, it completely disappeared. Now here i am on pregnancy number two (18 weeks) with PUPPPs again. Just bought dandelion tablets today and eagerly awaiting the postman to deliver my Rainer soap from internet. I hope it goes as quick as last time, it is so itchy. My heart goes out to out all this is unbelieveable, and so many doctors know nothing about it.
